The Blueballs Incident, or at least the Blueballs part, in addition to BF's nickname of "Agent Blueballs", is a reference to BF's blue balls. #MY TALKING HANK EASTER BUNNY PICTURE MOD#
This mod is one of the first mods to render BF and GF in an original artstyle. Also in Incident, Girlfriend shows some form of fear, though unlike in Ruv's case where GF is scared when he hits a note, or in cases like Whitty & A.G.O.T.I., where she's scared throughout the whole song, Smiler's instance shows her scared each time the screen shows glitches. During Rage, Redemption, and most portions in Incident, you can hear Smiler saying "oof". During Sadness, Smiler fittingly shows various forms of sadness, and semi-ironically in Rage, despite fittingly showing emotions of Rage, in the beginning and near-end of the song, he's frowning and smiling in each scenario respectively. Trollge (Smiler) is one of the first mods where the antagonist of a FNF mod changes expressions/forms mid-song, alongside, the now-trashed Sky, Pompom after her Week 2 update, and another Trollface/Trollge. (Also just as hard to notice during gameplay, the red and blue glitch effect present in his normal icon isn't there for his danger one.) 
Though considerably hard to notice during gameplay, Trollge's (Smiler) danger icon in Incident shows a slight difference, where he closes his jaw while still massively grinning, in contrast to his normal Incident icon, where both his grin is present, and his jaw is open. Trollge (Smiler) has the most forms out of any character, with a whopping 14. Tricky, suggesting a connection between them. The letter Derpina sends Boyfriend contains the Eye Symbol from Vs. The first was Kapi, the third being Flippy, and Alex Trollge (Smiler) is the second character to enter an angry phase, then leave it afterwards. At the end of the mod, when the player accesses some secret files, it is revealed that the Trollge that was your opponent is the same Trollge featured in in Malicious Bunny's "How to trollge your best friend", where the incident is identified as "Trollge Case 023 - The Smiler" and where that Trollge is named. However, only zardy is truly implied to be alive. This is one of the mods that ends with the opponent fading away, another being Garcello and Zardy. In the song Incident, you can see Malicious Bunny's videos playing in the background, albeit at a high speed. Only Sadness ends with a glitch effect, as Rage has a cutscene to end it, Incident has its version of Trollge (7), and Redemption ends with screen turning white, then black, then transitions into the credits. (See GameBanana page updates for further details.) The mod is confirmed to be getting a 'Phase T', in other words, a continuation of the Blueballs Incident. This mod is confirmed canon to the MaliciousBunny Trollge Universe and takes place directly after "How to trollge your best friend", this video created By Malicious Bunny explains all of Trollge Case 023 - The Smiler. In one part of Incident the health bar will push back, similar to Tricky the Clown's minion grabbing in Expurgation, however it can be countered by hitting notes, and it's slower, so it's not impossible. BF and GF smile as he says "thank you" before fading away. 
The screen fades to white after BF finishes his job and Smiler disappears. BF battles against the transformed Smiler, who at this point has become a Trollge, and after Boyfriend wins Smiler admits to his sins and asks BF to finish him off. He tells BF that there is no point in regretting anything and he asks to do "a little trolling".
Finally, he lets out a crooked smile, drops his axe and lets out a laugh. After the Troll lost, he questions why the two are still there and starts to get angry, taking out an axe during his second song. In the mod, Derpina calls on the Boyfriend to help take out Smiler, who is sad that he is not feeling sane (/sad he's not being remembered?). Smiler then takes out a picture of Derp next to a woman named Derpina and says he'll take care of her. After Derp is psychologically tortured, he grins madly and presumably goes insane. Derp questions where he is and tells the Trollge he is not his best friend, so Smiler tries to help him smile by showing him footage on a television which tells Derp to smile through subliminal messages. When Derp wakes up he is strapped to a chair and Smiler has a conversation with him. Smiler is sitting on a bench when he sees his "best friend" Derp and he follows him into an alleyway where he sneaks up behind him and knocks him out. In his final phase, he has the body of a normal Trollge but the head of his original self. Smiler constantly becomes glitched in numerous ways. However when pushed it seems he becomes more akin to a normal Trollge. Smiler starts out as a less monstrous Trollge, relating more to what a normal stick figure would look like.
